Can’t find what you are looking for? You can report on any other medicines, vaccines, devices, defective or falsified products (including fake coronavirus testing kits) via the Yellow Card website.
COVID-19 Vaccines
Vaccines are the most effective way to prevent infectious diseases and they save millions of lives worldwide. Like all medicines, vaccines can cause side effects. Most of these are mild and short-term, and not everyone gets them.
MHRA is responsible for continuously monitoring the safety of all medicines and vaccines once they are approved for use.
We have published a report summarising information received via the Yellow Card scheme which will be published regularly to include other safety investigations carried out by the MHRA under the COVID-19 Vaccine Surveillance Strategy.

Ventilator incident reporting
A dedicated phone line is available to report specifically for ventilators and respiratory support.
Call 0800 731 6789 for free between 9am and 5pm, Monday to Friday.
Outside of these hours please leave a message with your name and number, including as much information as you can about the device including the manufacturer's name. One of our team will get back to you.
Fake medicines and devices
For counterfeit or fake medicines or medical devices, including coronavirus testing kits, report as usual on the Yellow Card site.
